Application Guidelines and Requirements

A. REQUIREMENTS FOR APPLICATION or NOMINATION (6 copies each)

1. Application letter or recommendation letter with recommendee's conforme
2. Duly accomplished and notarized Personal Date Sheet (JBC FORM No. 1)
3. ID picture (2 x 2)
4. Certificate of Good Standing or latest official receipt from the IBP
5. Certification of Admission to the Bar (with Bar rating)
6. Income Tax Return for the two (2) preceding years
7. NBI Clearance
8. Transcript of Records
9. Sworn Medical Certificate issued, or results of an executive medical
examination conducted, within two (2) months prior to the filing of the
application or recommendation
10. Police clearance from place of residence

B. ADDITIONAL REQUIREMENTS

If from the government sector:
- Performance rating for the past three (3) years

If an incumbent Judge/Justice:
- Verified statement as to
a. Caseload
b. Average monthly output in all actions and proceedings
c. Number of cases deemed submitted for decision and date submitted
for decision
d. Number of decisions during the immediately preceeding 2- year-period
appealed to a higher court and number/percentage of affirmance thereof

If an incumbent prosecutor:
- Two (2) sample resolutions or opinions, and the informations thereon, if any,
filed thereafter.
- Verified statement as to
a. Number of pending preliminary investigations
b. Number of preliminary investigations submitted for resolution

If a private practitioner:
– Verified statement as to titles and case numbers of three (3) criminal cases
handled, as well as the branch numbers and stations of courts which tried the
same
– Sample complaint, memorandum, or appeal brief filed with the courts in the
said cases,
– Treatises, opinions, or articles written (if any)
